Your Pediatrician, ENT, Dentist, Orthodontist may refer you for speech-language, orofacial myofunctional therapy or you may want to contact me directly to begin a consult and/or assessment and treatment plan.

📝 Intake Paperwork & Prior Records
To ensure we provide the most personalized and effective care, you will be asked to complete comprehensive intake paperwork prior to or at the time of your initial visit. These forms include detailed questions about medical history, developmental milestones, communication concerns, and other relevant background information. This helps us gain a thorough understanding of you or your child’s unique needs and goals.
If speech-language therapy services have been provided in the past, please bring any relevant documentation with you—such as evaluation reports, progress notes, or Individualized Education Plans (IEPs). These records offer valuable insight and allow us to build upon previous interventions while maintaining continuity of care.

If you are covered by one of our accepted insurance plans above, we are happy to submit claims directly on your behalf as a courtesy. To ensure a smooth billing process, please verify that your coverage is active prior to your appointment and bring your current insurance card with you.
For patients whose insurance providers are not within our network, we are considered an out-of-network provider. However, we can furnish a detailed superbill upon request, which you may submit to your insurance company for potential reimbursement, depending on your plan’s out-of-network benefits. We recommend contacting your insurance provider in advance to understand your coverage and reimbursement options.
